The definition of the advanced practice nurse (APN) has evolved and varied throughout nursings history, but has seen its most targeted and specific ideas in recent years. As the needs of the populace prompt greater numbers of advanced nurse practitioners, the role and scope of practice for these nurses have broadened and converged to form an inclusive definition that works to advance the whole of nursing practice. The American Association of Colleges of Nursing (AACN), and the Advanced Practice Registered Nursing (APRN) consensus model, provide two definitions of the APN that showcase similarities and differences. Furthermore, the definition of the APN as presented in Advanced Practice Nursing: An Integrative Approach, by Hamric, Hansen, Tracy, & OGrady (2014), provides yet another consideration of what it means to be an APN. Each entity recognizes four specialties as part of advanced practice nursing: the nurse anesthetist, the nurse midwife, the nurse practitioner, and the clinical nurse specialist. Direct patient care is also a criterion that each perspective maintains is a critical component in defining the APN.
The AACN (2006) broadly defines advanced nursing practice as any nursing intervention that influences health care outcomes. In keeping with the traditional nursing model, these interventions include direct care of patients, as well as management and administration of nursing care, and developing and implementing health policy. Due to the increased autonomy of practice of the APN, assessment skills, specialized knowledge and a thorough understanding of pathophysiology and pharmacotherapeutics is expected. Finally, the AACN DNP Essentials recommends that nurses who practice at the professional level should receive doctoral level preparation. The AACN provides an overall arching definition for the APN; however, it does not provide specific measurable criteria as the consensus model does.
The Advanced Practice Registered Nurse (APRN) Consensus model provides a structured criterion by which to measure educational accomplishment of the APN. It standardizes APRN education, and focuses on measurable criteria (Thompson, 2011). Core classes provide the foundational education for the advanced practice nurse, with specialty knowledge incorporated, specific to the chosen field. Additionally, the consensus model promotes the doctor of nursing practice degree as entry into advanced nursing practice, similar to the AACN.
In Hamric, Hansen, Tracy, & OGradys (2014) Advanced Practice Nursing: An Integrative Approach, the definition of the APN is viewed as a concept, rather than a specific role, or skill set. Indeed, Hamric et al. (2014) provides a definition that is both conceptual and practical by identifying APN ideas, as well as regulatory and educational requirements. As in the previous definitions, direct patient care is central to the role of the APN, and while there is an educational core requirement, specific clinical skills vary as to the chosen specialty. Furthermore, while advanced practice nursing certainly involves advanced knowledge and skills, it is patient-focused and maintains the core nursing values in addition to an increased range of responsibilities. Essentially, Hamric (2014) incorporates the AACN DNP essentials, and the APRN consensus models, to produce a comprehensive definition of the APN.
Two factors most important, I believe, in Hamrics (2014) discussion of the APN, are that APNs expand their capability to provide and direct care, and the APNs role remains embedded in the nursing discipline. Ultimately, nurses are patient advocates who should continually seek to provide the best care for their patients. By seeking to expand their knowledge, and clinical practice, APNs can accomplish this for those in their charge. The concept that the role of the APN is intrinsically woven from the nursing discipline is another important factor. Grey (2012) points out that the APN enhances self-management and patient education, which are hallmarks of nursing education. The APN remains true to the original concept of nursing; yet, also expands upon the benefits and care that nursing brings to the general population, by seeking to improve outcomes.
