Explain the changes that occur to gastric acid stimulation and production with GERD, PUD, and gastritis disorders.

Jamie is a 3-month-old female who presents with her mother for evaluation of “throwing up.” Mom reports that Jamie has been throwing up pretty much all the time since she was born. Jamie does not seem to be sick. In fact, she drinks her formula vigorously and often acts hungry. Jamie has normal soft brown bowel movements every day and, overall, seems like a happy and contented baby. She smiles readily and does not cry often. Other than the fact that she often throws up after drinking a bottle, she seems to be a very healthy, happy infant. A more precise history suggests that Jamie does not exactly throw up—she does not heave or act unwell—but rather it just seems that almost every time she drinks a bottle she regurgitates a milky substance. Mom thought that she might be allergic to her formula and switched her to a hypoallergenic formula. It didn’t appear to help at all, and now Mom is very concerned.

Cases like these are not uncommon. The mother was concerned and thinking her daughter may have an allergy; she changed to a different formula. However, sometimes babies have immature GI tracts that can lead to physiology reflux as they adapt to normal life outside the uterus. Parents often do not consider this possibility, prompting them to change formulas rather than seeking medical care. As in the case study above, GI alterations can often be difficult to identify because many cause similar symptoms. This same issue also arises with adults—adults may present with symptoms that have various potential causes. When evaluating patients, it is important for the advanced practice nurse to know the types of questions he or she needs to ask to obtain the appropriate information for diagnosis. For this reason, you must have an understanding of common GI disorders such as gastroesophageal reflux disease (GERD), peptic ulcer disease (PUD), and gastritis.

To prepare: INCLUDE 2 CURRENT OUTSIDE RESOURCES PLUS THE ONES BELOW:

Review this week’s media presentation on the gastrointestinal system.

Review Chapter 33 in the Huether and McCance text. Identify the normal pathophysiology of gastric acid stimulation and production.

Review Chapter 35 in the Huether and McCance text. Consider the pathophysiology of gastroesophageal reflux disease (GERD), peptic ulcer disease (PUD), and gastritis. Think about how these disorders are similar and different.

Select a patient factor different from the one you selected in this week’s Discussion: genetics, gender, ethnicity, age, or behavior. Consider how the factor you selected might impact the pathophysiology of GERD, PUD, and gastritis. Reflect on how you would diagnose and prescribe treatment of these disorders for a patient based on this factor.

Review the “Mind Maps—Dementia, Endocarditis, and Gastro-oesophageal Reflux Disease (GERD)” media in the Week 2 Learning Resources. Use the examples in the media as a guide to construct a mind map for gastritis. Consider the epidemiology and clinical presentation of gastritis.

To complete:

Write a 3-page paper that addresses the following:

Describe the normal pathophysiology of gastric acid stimulation and production. Explain the changes that occur to gastric acid stimulation and production with GERD, PUD, and gastritis disorders.

Explain how the factor you selected might impact the pathophysiology of GERD, PUD, and gastritis. Describe how you would diagnose and prescribe treatment of these disorders for a patient based on the factor you selected.

Construct a mind map for gastritis. Include the epidemiology, pathophysiology, and clinical presentation, as well as the diagnosis and treatment you explained in your paper.

Describe one benefit and one limitation of involving the community in designing a survey and explain why.

From politics to marketing, from sociology to public health, surveys are routinely used for gathering information about an identified population. Surveys may cover a large swath of a population, such as the U.S. Census, or represent a microcosm of a larger population, such as a neighborhood public health survey.

In community health assessment, surveys are commonly used to explore an issue more widely than is possible through individual interviews or focus groups. Surveys may collect quantitative or qualitative data, or both. This week, you examine the use of surveys in community health assessment. Specifically, you consider the advantages and challenges of involving community members in both the design and implementation of surveys.

For this Discussion, review the Resources related to community involvement in survey design and implementation. Review the media titled Surveys. Consider some of the different ways that surveys were implemented and how the community was or was not involved in their design and implementation.

With these thoughts in mind:

3 pages of one advantage and one challenge of conducting community public health surveys and explain why. Describe one benefit and one limitation of involving the community in designing a survey and explain why. Then, describe one advantage and one challenge to involving the community in the implementation of a survey and explain why.

Be specific and use the Resources, media, and current literature to support your response.

Prepare a plan for utilizing Active Directory and Group Policy Objects to achieve the goals of setting secure permissions on business unit resources.

Suppose you were contracted by a local medium-sized business to speak with its business unit leaders about the importance of resource permissions in organizations. In your first moments on the job, you learn from a senior-level employee that the company just experienced a data theft incident at the hands of an employee, primarily due to the lack of permissions and resource segregation in the company. You later learn that there are multiple file servers on the network on which full control permissions are applied for the domain “Everyone” group.

Write a two to three (2-3) page paper in which you:

Explain the basic need for and concepts of file / folder permissions and business unit resource segregation in organizations so the company leaders will understand the importance of each.
Specify the strategy you would utilize for leveraging the business unit leaders to gain the necessary buy-in to implement your plan for resource and data segregation in the company.
Justify the use of NTFS permissions for the folders / files on the network and explain the top two to three (2-3) reasons this is the most reliable option at the business unit leaders’ disposal.
Prepare a plan for utilizing Active Directory and Group Policy Objects to achieve the goals of setting secure permissions on business unit resources.
Use at least three (3) quality resources in this assignment. Note: Wikipedia and similar Websites do not qualify as quality resources.

Your assignment must follow these formatting requirements:

Be typed, double spaced, using Times New Roman font (size 12), with one-inch margins on all sides; citations and references must follow APA or school-specific format. Check with your professor for any additional instructions.
Include a cover page containing the title of the assignment, the student’s name, the professor’s name, the course title, and the date. The cover page and the reference page are not included in the required assignment page length

Explain the type of data you plan touse for your dissertation, and how you plan to collect that data.

In the Week 7 Discussion, you conferred with colleagues and faculty in selecting an appropriate research method and design for your proposed research project. After you have chosen a research method for your dissertation, you are advised to continue to read other studies that apply this method. To clarify further that it is the best match for your own project, gather as much information as possible about how and when it is appropriately used.

Next, you must determine possible types of data you will gather for your research, possible sources of that data (e.g., public databases, published research results, surveys, interviews, etc.), and potential ways to analyze that data. The type of data you obtain and how you analyze those data is often stipulated by the research method you chose. If, for example, you chose a quantitative method, you will use statistics (e.g., MANOVA, logistical regression, etc.) to analyze your data. The statistics you use will depend on the type of data (nominal, ordinal, interval, or ratio) and number of variables in your study. If you chose a qualitative method, your analysis might consist of organizing descriptive information into a story, creating concepts, identifying themes, or certain types of statistical analyses.

Note: The inclusion of possible analytical strategies is optional in your draft Prospectus, but it is an important dissertation consideration at this time.

2-3 pages of a brief description of the research method you selected. Explain the type of data you plan touse for your dissertation, and how you plan to collect that data. Describe a type of analysis (t-tests, ANOVA, Chi-square, theming, concept analysis, etc.) you might use to analyze your data and explain the reason(s) for your selection. Be specific and support your selections with references to the literature
